REGISTRATION FORM NOTES

( To be read in conjunction with the Registration Form before filling out )

Registration Fees:  

1. Payment: Please make cheques payable to VALID (Note: credit card facilities are NOT available) 
   and forward with your registration form to: VALID, 235 Napier Street, Fitzroy 3065
    

2. Daily Registration Fees include:
   
      BBQ lunch on Friday for all Friday delegates.
      Morning and afternoon teas each day.
      Entrance to Conference sessions and activities. (eg; Come & Try Activities)
   NOTE: Individual day registration for Thursday, does NOT include registration for the Disco/Dinner.
   
   

3. Registration Fees DO NOT include:

      Drinks at the Dinner Disco.  (Note: drinks can be bought at bar prices)
      Meals other than the Conference Dinner on Thursday Night and Friday lunch BBQ.
      Transport to and from the venue.
      A copy of the Conference video is not included.  The video will be available for
         purchase at a later date, from the VALID office.
      Individual day registration for Thursday, does not include registration for the
         Disco/Dinner.

4. Whole Conference' Registration Fee includes items in point 2, plus entrance on Thursday evening
   to the Conference Dinner which includes a two course meal and Disco/Chat.
   

5. The Early Bird registration fees apply only to registrations received and paid by the 
   23rd November, 2007.  From the 24/11/2007 'Standard Registration' fees will apply to all registrations.
   

6. 'Concession' Registration Fee is applicable to individuals who have a Health Care Card or 
   Pensioner Concession Card.  Additional Sponsorship can be applied for (use Sponsorship Application Form) where people are experiencing significant financial hardship.  Please note 
   that if applying for sponsorship for registration costs, do not send payment until notified of outcome.
   
   

7. The reduced fee for 'Support Staff' category is only available to staff or carers who are providing assistance (eg. transport, personal care, supervision) during the Conference for individuals with a disability who are unable to participate without this support.

 

8. Membership discount applies to individuals or agencies who are current paid VALID members.
   

9. Registration Fees are only refundable where unexpected circumstances occur and where written advice is provided to VALID by 25th January, 2008.   After this date, fees are not refundable.
   

10. Registration Fees are GST free.
    

11. All Registrations close on the 25th January, 2008.

VALID Members Discount

Current paid members (individuals and agencies) of VALID can receive a 10% discount off their Registration Fee.  To become a VALID member please contact the office or download a Membership Form from our website.

Registration Payment Methods

• VALID accepts cheques, money orders and direct payments.

• VALID does not have Credit card Payment facilities.

Photos:

The permission given on the registration form is only for photos and video footage of ordinary Conference activities.  All shots will be respectful, promote dignity and are only used for Conference publicity.
